11-Hydroxyandrostenedione Test for Adrenal Gland Function

Veröffentlicht: 9. August 2026Aktualisiert: 15. August 2026

This blood test measures the level of 11-Hydroxyandrostenedione, a hormone produced by the adrenal glands, after they have been stimulated. It helps your doctor assess how well your adrenal glands respond to a corticotropin injection and can be useful in diagnosing several hormonal disorders.

What is this test?

The 11-Hydroxyandrostenedione test measures the amount of a hormone called 11-Hydroxyandrostenedione in your blood. Your adrenal glands are small glands located on top of each kidney, and they produce this hormone along with other important hormones. To stimulate them, a healthcare professional gives you an injection of corticotropin, also called ACTH. Your blood is then collected exactly 1 hour later to show how your adrenal glands responded to the stimulation.

Why do I need this test?

Your doctor may order this test when they suspect that your adrenal glands are not functioning normally. It can help diagnose congenital adrenal hyperplasia, a genetic condition that affects hormone production, as well as adrenal insufficiency, which occurs when the adrenal glands do not produce enough hormones. The test may also identify other adrenal disorders and help distinguish between different types of adrenal gland problems.

How should I prepare for this test?

Follow your doctor's instructions carefully before the test. Depending on the laboratory's requirements, you may need to fast, meaning that you do not eat or drink anything except water for a specified period. Tell your doctor about every medication, supplement, and herbal product you are taking, because some can affect the results. If necessary, your doctor will advise you to stop certain medicines beforehand. Wear comfortable clothing with sleeves that can easily be rolled up for the blood draw.

What happens during the test?

First, a healthcare professional will inject corticotropin, also called ACTH, into a muscle or a vein. You will then wait for exactly 1 hour before a blood sample is collected from a vein in your arm with a small needle. The blood draw usually takes only a few minutes. You may feel a brief pinch, sting, or scratching sensation when the needle enters your skin. After the procedure, you can generally return to your normal activities unless your healthcare provider gives you different instructions.

Are there any risks?

The risks of this test are generally minimal. You may notice slight pain, bruising, swelling, or minor bleeding at the injection or blood draw site. Some people feel lightheaded during or after the blood draw, particularly if they have not eaten or have had little fluid. The corticotropin injection may temporarily cause facial flushing, an increased heart rate, headache, dizziness, or mild nausea. These effects usually resolve quickly, although serious reactions to the injection are rare.

Reference range

Reference range: 3.0-15.0 nmol/L at 1 hour post 250 mcg ACTH stimulation. Normal response shows significant increase from baseline. Peak levels typically occur 30-60 minutes post-stimulation.N/A

Probenart: serum

Patient should rest for 30 minutes prior to test. Baseline sample collected, then 250 mcg corticotropin (ACTH) administered intravenously or intramuscularly. Second sample collected exactly 60 minutes post-administration. Inform patient about potential for mild flushing or dizziness during test.

Clinical interpretation

How abnormal results are interpreted and what conditions they suggest.

How long does the test take?
The entire procedure usually takes about 1-2 hours. The corticotropin injection is given first, followed by a waiting period of exactly 1 hour before your blood is drawn. The blood collection itself takes only a few minutes, but additional time may be needed for check-in, preparation, and observation afterward.
When will I get my results?
Results are usually available within a few days to one week, depending on the laboratory that processes your sample. Your doctor may contact you when the results are ready or discuss them with you at your next scheduled appointment. If you have not received your results within the expected timeframe, contact your doctor's office for an update.
Do I need to fast before this test?
Follow the specific instructions provided by your doctor or laboratory. Some laboratories require fasting for a certain period, while others do not. If you are unsure whether you should eat or drink beforehand, ask your healthcare provider for clear instructions. You should never fast for a prolonged period unless you have been specifically instructed to do so.
What do abnormal results mean?
Abnormal results may indicate an adrenal gland disorder, but they do not establish a diagnosis on their own. Your doctor will interpret your results in relation to your symptoms, medical history, physical examination, and other laboratory findings. Depending on what the results show, additional testing may be needed to confirm the cause and recommend appropriate treatment.
Can I take my regular medications before the test?
Tell your doctor about all medications you are taking, including prescription medicines, over-the-counter products, supplements, and hormone treatments. Some medicines, especially steroids or other hormonal therapies, can alter hormone levels and affect the test results. Do not stop any medication on your own. Your doctor will tell you which medicines to pause, which to continue, and when to resume them after the test.
