How is the test performed?

A blood sample will be drawn from you (the recipient) and also from the potential donor. Your serum is mixed with the donor's B cells in a laboratory. Technicians then look for signs of reaction, such as cell damage or death, which would indicate the presence of donor-specific antibodies. The test result is reported as either positive (antibodies present and reactive) or negative (no detectable reactive antibodies).

What do the results mean?

Reference range

Negative (No donor-specific antibodies detected on B lymphocytes). Positive crossmatch indicates presence of donor-reactive antibodies and is generally a contraindication to transplantation.N/A

検体: whole-blood

No fasting required. Patients should inform the laboratory of all medications, particularly immunosuppressants, rituximab, or recent blood transfusions (within 14 days) which may affect results. Recent plasmapheresis or IVIG administration should be documented.

Clinical interpretation

How abnormal results are interpreted and what conditions they suggest.

Why is the B cell crossmatch important before a transplant?
This test helps prevent hyperacute rejection, which occurs when your immune system immediately attacks the transplanted organ. If you have pre-formed antibodies against the donor's cells, your body would recognize the new organ as foreign and mount a rapid, destructive immune response that could cause the transplant to fail within hours.
Does a positive crossmatch mean I can never receive a transplant?
Not necessarily. A positive crossmatch indicates higher risk, but there are desensitization protocols that can help reduce antibody levels, making transplantation possible. Your transplant team will discuss options which may include waiting for a different donor, undergoing treatments to lower antibodies, or participating in specialized programs for sensitized patients.
How long does it take to get crossmatch results?
The test typically takes several hours to a few days to complete, depending on the laboratory and the specific testing method used. For urgent transplant situations, some centers can provide results more quickly. Your transplant coordinator will inform you of the expected timeline.
Will I need this test repeated if I'm on a transplant waiting list?
Yes, a crossmatch must be performed for EACH potential donor offer. You cannot use an old crossmatch result because your antibody status can change over time due to transfusions, infections, or other exposures. Each new donor candidate requires a fresh crossmatch test.
Is this test different from a blood type matching test?
Yes, these are separate tests. Blood type matching (ABO compatibility) is the first basic requirement for transplantation. The B cell crossmatch is a much more sensitive test that detects specific antibodies against donor cells at the cellular level, providing additional safety beyond blood type matching alone.
