Alpha-Fetoprotein-L3, Serum/Plasma — Immunoassay, Mass Concentration

Published: August 9, 2026Updated: August 16, 2026

Measures the AFP-L3 form of alpha-fetoprotein in blood. It may be used as an additional marker in evaluating liver disease or hepatocellular carcinoma risk.

What is this test?

Alpha-fetoprotein-L3, or AFP-L3, is a form of alpha-fetoprotein, a protein produced mainly by the liver in developing fetuses. This test measures AFP-L3 in serum or plasma. It is different from a standard total alpha-fetoprotein test.

Why might I need this test?

AFP-L3 may be ordered as an additional test when a clinician is evaluating a liver mass, persistent liver abnormalities, or the risk of hepatocellular carcinoma. It is generally interpreted together with imaging, liver function tests, and other clinical findings rather than used alone.

How is the test performed?

A blood sample is drawn and analyzed in the laboratory using an immunoassay. The result reports the amount of AFP-L3 present in the blood.

How do I prepare for the test?

This test usually requires no special preparation. Follow any instructions from your clinician or laboratory, and tell them if you are pregnant because alpha-fetoprotein levels can naturally change during pregnancy.

Frequently asked questions

Is AFP-L3 the same as a total AFP test?
No. Total AFP measures all circulating alpha-fetoprotein, while this test specifically measures the AFP-L3 form. A total AFP result may sometimes be ordered alongside it.
Does an abnormal AFP-L3 result mean I have liver cancer?
No. Elevated levels can occur in several conditions, including other forms of liver disease. Imaging and additional tests are often needed to determine the cause.
Do I need to fast before this test?
Fasting is usually not required, but preparation instructions can vary by laboratory. Confirm the requirements with the ordering clinician or testing laboratory.
