###1: Lactate dehydrogenase (LDH), catalytic concentration, serum or plasma

Published: August 9, 2026Updated: August 17, 2026

###3: This blood test measures the activity of lactate dehydrogenase, an enzyme found in many tissues. A high or low result can help evaluate conditions affecting the heart, liver, muscles, blood cells, or other organs.

Quick summary

["###4: LDH is a nonspecific marker of tissue injury, so it is most useful when interpreted with symptoms, examination findings, and other tests."]

###5: What this test measures

###6: LDH helps convert lactate to pyruvate during energy production. Because the enzyme is present in the heart, liver, skeletal muscle, red blood cells, and other tissues, an abnormal level can reflect injury somewhere in the body.

###7: How the test is performed

###8: A blood sample is collected, and the serum or plasma is analyzed for LDH activity. The laboratory may use an enzymatic method, depending on the testing laboratory. You generally do not need to fast unless your clinician gives different instructions.

###9: Why clinicians order it

###10: LDH may be ordered as part of an evaluation for chest pain, suspected heart or liver injury, muscle symptoms, anemia, or other unexplained symptoms. It is often interpreted alongside tests such as CK, AST, ALT, and cardiac markers.

###11: Factors that can affect the result

###12: Strenuous exercise, muscle injury, injections, hemolysis during sample collection, and certain medicines can alter LDH activity. The sample should be handled carefully because breakdown of red blood cells in the tube can falsely raise the result.

Frequently asked questions

###14: Does a high LDH level mean a heart attack?
###15: Not necessarily. LDH can rise with injury in several organs, including the liver and muscles, and it is not specific for the heart. Cardiac troponin and other clinical information are generally more useful when evaluating a suspected heart attack.
###16: Can this test show where the problem is?
###17: No. LDH is a broad marker of tissue injury or increased cell turnover. Your clinician may use additional tests and your medical history to determine the cause and location of the problem.
###18: What if my result is slightly outside the laboratory's reference interval?
###19: A mild difference may result from exercise, muscle activity, sample handling, or normal biological variation and may not be clinically important. Discuss persistent or substantial abnormalities with your clinician rather than interpreting the result on its own.
