Allopurinol / Probenecid

Published: July 27, 2026Updated: August 27, 2026

Allopurinol/Probenecid is a combination medication containing two agents that lower uric acid through complementary mechanisms. It is prescribed for chronic gout and hyperuricemia when single-agent therapy is not adequately controlling uric acid levels. This combination helps reduce gout attacks, dissolve tophi, and prevent uric acid-related kidney complications.

Quick summary

A dual-mechanism uric acid-lowering medication for chronic gout when single agents are insufficient.

Treatment of chronic gout and hyperuricemia, particularly when monotherapy with either allopurinol or probenecid alone does not provide adequate control. Used to prevent gout attacks, reduce tophi formation, and decrease risk of uric acid kidney stones.

Allopurinol inhibits xanthine oxidase, an enzyme involved in uric acid production, thereby reducing uric acid synthesis. Probenecid increases renal excretion of uric acid by blocking its reabsorption in the kidney tubules. Together, these agents lower serum uric acid through both reduced production and enhanced elimination.

Requires adequate hydration (2-3 liters daily) to reduce kidney stone risk. May initially trigger gout attacks during initiation—prophylactic colchicine is often prescribed. Not recommended in patients with renal impairment, history of kidney stones, or in those with uric acid overproduction. Requires regular monitoring of serum uric acid and renal function.
