Treatment of type 2 diabetes mellitus in adults when glycemic control is inadequate with single-agent therapy or dual therapy with alogliptin or metformin alone.
Alogliptin inhibits the DPP-4 enzyme, increasing levels of incretin hormones that stimulate insulin release in response to meals; metformin decreases hepatic glucose production and improves insulin sensitivity in peripheral tissues.
Not indicated for type 1 diabetes or diabetic ketoacidosis; metformin component carries risk of lactic acidosis (contraindicated in severe renal impairment); regular monitoring of renal function recommended; may cause gastrointestinal side effects particularly when initiating therapy; hypoglycemia risk increases when combined with insulin or insulin secretagogues.
