Alprostadil is used in adult men with erectile dysfunction who have not responded to oral therapies, and in neonates with ductus arteriosus-dependent congenital heart lesions. It may also be used for peripheral vascular disorders such as Buerger's disease and severe Raynaud's phenomenon.

Alprostadil acts directly on vascular smooth muscle receptors to cause relaxation and vasodilation. This increases blood flow to the affected area, whether in the penile tissue for erectile function, the ductus arteriosus to maintain patency, or peripheral vessels to improve circulation.

Alprostadil should not be used in patients with conditions that predispose to priapism, bleeding disorders, or known hypersensitivity. It is administered via intracavernosal injection or urethral suppository for erectile dysfunction, and intravenously or intra-arterially for other indications. Patients should be properly trained on administration technique and seek immediate medical attention for prolonged erections.
