Amiloride / Hydrochlorothiazide / Methyldopa
Amiloride/hydrochlorothiazide/methyldopa is a combination antihypertensive medication used to treat high blood pressure. It combines three agents: a thiazide diuretic to reduce fluid volume, a potassium-sparing diuretic to prevent excessive potassium loss, and a centrally acting agent to reduce sympathetic nervous system activity. This combination may be prescribed when single agents or simpler combinations are insufficient for blood pressure control.
A triple-combination antihypertensive that lowers blood pressure through complementary mechanisms including fluid reduction, potassium preservation, and reduced sympathetic activity.

Primarily used for the treatment of hypertension (high blood pressure), particularly when blood pressure is not adequately controlled with fewer medications. The combination may be especially useful in patients who need multiple agents to achieve target blood pressure goals.
Hydrochlorothiazide inhibits sodium reabsorption in the kidney tubules, promoting water loss and reducing blood volume. Amiloride works in the collecting duct to prevent potassium secretion, counteracting the potassium-depleting effect of thiazides. Methyldopa is converted to alpha-methylnorepinephrine in the brain, where it acts as a false neurotransmitter and reduces sympathetic outflow, leading to decreased peripheral vascular resistance.
Requires regular monitoring of blood pressure, electrolytes, and kidney function. Patients should be aware of potential side effects including dizziness, electrolyte imbalances, and sedation (particularly from methyldopa). Alcohol and other sedatives may enhance drowsiness. Patients should not discontinue this medication abruptly without medical supervision. This medication may interact with other drugs including lithium, digoxin, and certain antipsychotics. Always disclose all medications and supplements to your healthcare provider.
