This medication is primarily used for moderate to severe anxiety disorders, particularly when symptoms of anxiety are accompanied by agitation, tension, or depressive features. It may also be prescribed for certain psychotic disorders and for short-term management of severe anxiety that hasn't responded to other treatments.
The three components work together: amitriptyline increases certain neurotransmitters in the brain to improve mood; diazepam enhances the effect of GABA to reduce anxiety and promote calmness; and perphenazine blocks dopamine receptors to reduce agitation and psychotic symptoms. Together they provide rapid anxiolytic effects with longer-term mood stabilization.
Due to the diazepam component, this medication carries risk of dependence with prolonged use and should not be stopped abruptly. It may cause drowsiness and impair alertness, particularly affecting ability to drive or operate machinery. Alcohol should be avoided. This combination is typically used when single-agent therapies have been insufficient and requires close monitoring by a healthcare provider.
