Arikayce is indicated for the treatment of Mycobacterium avium complex (MAC) lung disease in adults who have not achieved negative sputum cultures after at least 6 months of a guideline-based multidrug regimen (partial treatment regimen). It is used as part of a combination therapy with other antibiotics.

Arikayce contains amikacin, an aminoglycoside antibiotic, encapsulated in liposomes that are inhaled directly into the lungs. The liposomal formulation allows for sustained release and improved delivery to the site of infection. Amikacin works by inhibiting bacterial protein synthesis, effectively killing the MAC bacteria.

Common side effects include cough, dyspnea, bronchospasm, and dysphonia. Arikayce carries a Boxed Warning for increased risk of respiratory adverse reactions and ototoxicity (hearing loss). It should be used with caution in patients with renal impairment and requires regular monitoring. The medication is administered once daily via a specialized nebulizer device.
