Avibactam / Aztreonam

Published: July 27, 2026Updated: August 27, 2026

Avibactam/aztreonam is a combination antibiotic used to treat serious gram-negative bacterial infections, particularly those caused by organisms resistant to standard antibiotics. This combination is especially valuable for infections involving bacteria that produce certain beta-lactamase enzymes, which would otherwise render many antibiotics ineffective. It is typically reserved for complex infections such as complicated intra-abdominal infections, urinary tract infections, and hospital-acquired pneumonia when resistant organisms are suspected or confirmed.

Quick summary

A combination antibiotic pairing a beta-lactamase inhibitor with a monobactam to treat resistant gram-negative bacterial infections.

Treats complicated intra-abdominal infections, complicated urinary tract infections, and hospital-acquired/ventilator-associated bacterial pneumonia caused by susceptible gram-negative bacteria. Often used when infections involve multi-drug resistant organisms or when standard antibiotics have failed.

Aztreonam is a monobactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell death. Avibactam is a beta-lactamase inhibitor that protects aztreonam from degradation by certain beta-lactamase enzymes, thereby extending its spectrum of activity against resistant bacteria.

Administered intravenously. Reserved for use when other treatments are inappropriate due to resistance or safety concerns. Monitor for allergic reactions, particularly in patients with known penicillin or cephalosporin allergies (cross-reactivity risk is low but exists). May cause side effects including gastrointestinal disturbances, liver enzyme elevations, and infusion reactions. Complete the full prescribed course even if symptoms improve.
