Besponsa is indicated for the treatment of adults with relapsed or refractory B-cell precursor acute lymphoblastic leukemia (ALL). It is typically used in patients who have undergone prior therapy.

Besponsa is an antibody-drug conjugate that consists of a monoclonal antibody targeting the CD22 protein (found on B-cell surfaces) linked to a cytotoxic agent (ozogamicin). When the antibody binds to CD22 on cancer cells, the drug is internalized and releases its cytotoxic payload, causing DNA damage and triggering cell death.

Besponsa carries a boxed warning for severe hepatotoxicity, including fatal liver failure and veno-occlusive disease/sinusoidal obstruction syndrome. Common side effects include thrombocytopenia, neutropenia, anemia, leukopenia, fatigue, fever, nausea, headache, and liver enzyme elevations. It may cause infusion-related reactions and QT prolongation. The medication is not recommended during pregnancy as it can cause harm to an unborn baby.
