Bexagliflozin is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It is not indicated for use in patients with type 1 diabetes.
Bexagliflozin inhibits SGLT2, the transporter responsible for reabsorbing approximately 90% of filtered glucose from the renal glomerular filtrate in the proximal tubule. By blocking this transporter, bexagliflozin reduces renal glucose reabsorption, thereby lowering blood glucose levels through increased urinary glucose excretion.
Common side effects include genital mycotic infections (particularly in women) and urinary tract infections. Patients should be monitored for signs of volume depletion, hypotension, and ketoacidosis. The medication may increase LDL cholesterol. Adequate hydration is recommended to reduce the risk of adverse effects.
