Bivalirudin Trifluoroacetate

Published: July 26, 2026Updated: August 24, 2026

Bivalirudin Trifluoroacetate is a direct thrombin inhibitor anticoagulant used primarily during percutaneous coronary intervention (PCI) procedures and in patients with unstable angina. It provides anticoagulation by reversibly binding to both free and clot-bound thrombin, preventing blood clot formation. This medication is administered intravenously under medical supervision in hospital settings.

Quick summary

Bivalirudin Trifluoroacetate is an intravenous anticoagulant used to prevent blood clots during heart procedures.

Molecular mechanism of anticoagulation

Used as an anticoagulant for patients undergoing percutaneous coronary intervention (PCI), including those with ST-segment elevation myocardial infarction (STEMI), non-ST-segment elevation acute coronary syndrome (NSTE-ACS), and unstable angina. Also used as an alternative to heparin in patients at risk of heparin-induced thrombocytopenia.

A synthetic peptide that directly inhibits thrombin (Factor IIa) by reversibly binding to both the active site and exosite 1 of the thrombin molecule, thereby preventing conversion of fibrinogen to fibrin and inhibiting thrombin-induced platelet aggregation.

Administered intravenously with dosing based on weight and renal function. Requires careful monitoring of activated clotting time (ACT) during procedures. Contraindicated in patients with active major bleeding or hypersensitivity. May increase bleeding risk and should be used with caution in patients with renal impairment. Reversible action allows for predictable recovery of coagulation.
