Blenrep is indicated for the treatment of adult patients with relapsed or refractory multiple myeloma who have received at least four prior lines of therapy, including a proteasome inhibitor, an immunomodulatory agent, and an anti-CD38 monoclonal antibody.

Blenrep is a B-cell maturation antigen (BCMA)-directed antibody-drug conjugate. The monoclonal antibody component binds to BCMA on the surface of multiple myeloma cells. The conjugate is then internalized, releasing the cytotoxic agent (mafodotin) inside the cell, which disrupts microtubule function and leads to cell death.

Blenrep carries a boxed warning for ocular toxicity, including keratopathy (corneal deposits) and changes in visual acuity. Regular ophthalmologic examinations are required before and during treatment. Common side effects include thrombocytopenia, anemia, infusion-related reactions, and blurred vision. This medication is administered intravenously by a healthcare professional.
