Brimonidine / Carbachol

Published: July 27, 2026Updated: August 28, 2026

Brimonidine/Carbachol is an ophthalmic combination medication used primarily to reduce intraocular pressure in patients with glaucoma or ocular hypertension. This combination works through complementary mechanisms: brimonidine decreases aqueous humor production while carbachol increases outflow through the trabecular meshwork. The dual-action approach provides more effective pressure reduction than either agent alone.

Quick summary

An eye drop combination for glaucoma that lowers pressure by decreasing fluid production and increasing fluid drainage.

Treatment of elevated intraocular pressure in open-angle glaucoma and ocular hypertension. May be used when monotherapy is insufficient for adequate pressure control.

Brimonidine is an alpha-2 adrenergic agonist that reduces aqueous humor production. Carbachol is a cholinergic agonist that increases uveoscleral outflow and causes pupillary constriction. Together, they provide complementary pressure-lowering effects.

For ophthalmic use only. May cause temporary blurred vision, eye irritation, or redness. Systemic absorption is possible; monitor for side effects including hypotension or bradycardia. Contraindicated in patients with angle-closure glaucoma. Avoid contact lens wear immediately after administration.
