Essential hypertension (high blood pressure) when monotherapy or dual therapy is insufficient. May be considered when blood pressure requires multi-targeted approach for adequate control.

Buthiazide (thiazide diuretic) reduces fluid volume by increasing urinary sodium excretion; Dihydralazine relaxes arterial smooth muscle to reduce peripheral vascular resistance; Metipranolol (beta-blocker) decreases heart rate and cardiac output. Together they lower blood pressure through complementary mechanisms.

May cause electrolyte imbalances (especially low potassium), dizziness, or fatigue. Contraindicated in severe bradycardia, heart block, heart failure, and severe renal impairment. Avoid abrupt discontinuation of beta-blocker component. Regular monitoring of blood pressure, electrolytes, and renal function recommended. May interact with other antihypertensives, lithium, or NSAIDs.
