Bylvay is indicated for the treatment of pruritus (itching) in patients 3 months of age and older with progressive familial intrahepatic cholestasis (PFIC). PFIC is a rare genetic disorder that causes progressive liver disease and impairs bile flow, leading to bile acid buildup that causes severe itching and liver damage.

Bylvay is an ileal bile acid transporter (IBAT) inhibitor. It acts locally in the small intestine to block the uptake of bile acids from the gut, reducing the amount of bile acids returning to the liver. By interrupting this enterohepatic circulation, it helps lower bile acid levels in the bloodstream, which is believed to be the mechanism that reduces the itching associated with PFIC.

Bylvay is taken once daily with a meal. Common side effects may include diarrhea, stomach pain, and weight gain. Liver function should be monitored during treatment. It is contraindicated in patients with evidence of extrahepatic bile duct obstruction. The capsules may be opened and sprinkled on soft food for patients who have difficulty swallowing. Patients should consult their healthcare provider for complete safety information and appropriate monitoring.
