Treatment of invasive aspergillosis (in patients who are refractory to or intolerant of other antifungal therapies), candidemia, esophageal candidiasis, and empiric therapy for presumed fungal infections in febrile, neutropenic patients.
Caspofungin is an echinocandin that inhibits beta-(1,3)-D-glucan synthase, an enzyme essential for fungal cell wall synthesis. This disrupts cell wall integrity, causing fungal cell lysis and death.
Administered by slow IV infusion only. Common side effects include fever, headache, nausea, and flushing. Requires dose adjustment in hepatic impairment. May interact with cyclosporine, tacrolimus, and certain anticonvulsants. Should not be used in patients with known hypersensitivity to echinocandins.
