Treatment of hospital-acquired pneumonia (HAP) and community-acquired pneumonia (CAP) caused by susceptible bacteria, including methicillin-resistant Staphylococcus aureus (MRSA) and other drug-resistant organisms. May also be used for skin and soft tissue infections.

As a prodrug, it is converted to its active form, ceftobiprole, which inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). It has particular affinity for PBP2a, allowing it to be effective against MRSA and other resistant Gram-positive bacteria while also covering many Gram-negative pathogens.

Administered via intravenous infusion. Contraindicated in patients with known hypersensitivity to cephalosporins or severe penicillin allergies. Requires monitoring for potential adverse effects including seizures, hypersensitivity reactions, and C. difficile-associated diarrhea. Susceptibility testing is recommended before use to ensure appropriate targeting of susceptible organisms.
