Chenodal is primarily used to treat radiolucent (non-calcified) cholesterol gallstones in patients who cannot undergo surgery, and it is also approved for cerebrotendinous xanthomatosis (CTX), a rare inherited disorder characterized by abnormal lipid accumulation in tissues.

As a bile acid, Chenodal reduces cholesterol secretion into bile and may inhibit intestinal cholesterol absorption, creating a bile composition that helps dissolve existing cholesterol gallstones over time. In CTX, it helps reduce the accumulation of cholestanol, a cholesterol-like substance that causes tissue damage.

Chenodal is available only through a restricted distribution program due to potential hepatotoxicity. Regular liver function monitoring is required during treatment. It is contraindicated in patients with known liver disease, bile duct obstruction, or radiopaque stones. Common side effects may include diarrhea and elevated liver enzymes.
