Clorazepic acid is indicated for the treatment of anxiety disorders, including generalized anxiety and panic disorder, and is used as an adjunct in the management of acute alcohol withdrawal to reduce associated symptoms such as agitation and seizures.
As a prodrug, clorazepic acid is metabolized in the stomach to desmethyldiazepam, which then binds to benzodiazepine receptors on the GABA-A receptor complex. This enhances the inhibitory effects of GABA, resulting in decreased neuronal excitability and producing sedative, anxiolytic, muscle relaxant, and anticonvulsant effects.
This medication carries a risk of dependence and withdrawal with prolonged use. It may cause drowsiness and impair cognitive function, so activities requiring alertness should be avoided. Combination with alcohol, opioids, or other CNS depressants significantly increases the risk of severe respiratory depression. Use during pregnancy is generally contraindicated due to potential fetal harm.
