Codeine Phosphate Anhydrous is an opioid analgesic used for the treatment of mild to moderate pain and as a cough suppressant. It works by binding to opioid receptors in the brain and central nervous system, where it is metabolized to morphine to produce its analgesic and antitussive effects. Mild to moderate pain relief, cough suppression (particularly dry, persistent coughs), and occasionally for diarrhea management. Often combined with other medications such as paracetamol or ibuprofen in fixed-dose formulations.

Binds to mu-opioid receptors in the brain and spinal cord, reducing the perception of pain and suppressing the cough reflex. It is a prodrug that is metabolized to morphine and other active metabolites via the CYP2D6 enzyme pathway.

Carries risks of dependence, tolerance, and respiratory depression especially at higher doses. Use with caution in patients with respiratory conditions, hepatic/renal impairment, or history of substance abuse. Not recommended during breastfeeding due to risk of infant sedation. CYP2D6 poor metabolizers may experience reduced efficacy, while ultra-rapid metabolizers may experience enhanced effects and increased risk of toxicity.
