Cosela (trilaciclib) is a prescription medication used to reduce the risk of bone marrow suppression in adults with extensive-stage small cell lung cancer (ES-SCLC) receiving certain chemotherapy regimens. It works by protecting blood cell precursors from chemotherapy damage, helping to prevent low blood counts and related complications. This medication is administered as an intravenous infusion before chemotherapy sessions.

Cosela is indicated to reduce the incidence of chemotherapy-induced myelosuppression (low blood cell counts) in adults with extensive-stage small cell lung cancer who are receiving carboplatin and etoposide chemotherapy regimens.
Cosela is a cyclin-dependent kinase 4/6 (CDK4/6) inhibitor that temporarily halts bone marrow stem and progenitor cells in the G1 phase of the cell cycle. Since chemotherapy primarily targets rapidly dividing cells, this protective arrest helps shield blood cell precursors from chemotherapy-induced damage.
Cosela is administered as a 30-minute intravenous infusion within 4 hours before chemotherapy. Common side effects may include fatigue, headache, and injection site reactions. It is specifically approved only for extensive-stage small cell lung cancer and is not indicated for other cancer types. Patients should discuss their complete medical history with their healthcare provider, including any known allergies, before starting treatment.
