Crofelemer is indicated for the symptomatic relief of non-infectious diarrhea in adults with HIV/AIDS who are receiving antiretroviral therapy. It has also been studied for use in diarrhea-predominant irritable bowel syndrome (IBS-D) and other conditions involving secretory diarrhea.
Crofelemer inhibits both the cyclic adenosine monophosphate (cAMP)-stimulated cystic fibrosis transmembrane conductance regulator (CFTR) chloride channel and calcium-activated chloride channels (CaCC) located at the luminal membrane of enterocytes. By blocking these channels, it reduces chloride secretion into the intestinal lumen, which decreases water secretion and slows intestinal transit, resulting in reduced diarrhea.
Crofelemer is a locally acting medication with minimal systemic absorption. It should be taken twice daily with or without food. Patients should be advised that this medication treats symptomatic diarrhea but does not cure any underlying infection. Common side effects may include upper respiratory tract infections, cough, and flatulence. Patients should consult their healthcare provider if diarrhea persists or worsens.
