Defibrotide Sodium is an antithrombotic and fibrinolytic agent derived from bovine pulmonary DNA. It is primarily used to treat severe hepatic veno-occlusive disease (sinusoidal obstruction syndrome) in patients undergoing hematopoietic stem cell transplantation. The medication works by restoring fibrinolytic balance and reducing endothelial cell damage in hepatic blood vessels.

Quick summary

Defibrotide Sodium is used to treat severe veno-occlusive disease of the liver following stem cell transplantation by promoting fibrinolysis and protecting the vascular endothelium.

Treatment of severe hepatic veno-occlusive disease (VOD), also known as sinusoidal obstruction syndrome (SOS), in hematopoietic stem cell transplantation (HSCT) patients. It is specifically indicated for patients with renal or pulmonary dysfunction in addition to VOD.

Defibrotide Sodium exhibits antithrombotic, anti-ischemic, and anti-inflammatory properties. It increases tissue plasminogen activator (tPA) and thrombomodulin expression while decreasing plasminogen activator inhibitor-1 (PAI-1) and von Willebrand factor, restoring fibrinolytic balance and protecting the vascular endothelium from damage.

Major risk is hemorrhage, particularly in patients with compromised integrity of blood vessels. Contraindicated with concomitant systemic anticoagulant or thrombolytic therapy. Should be discontinued 2-4 hours prior to invasive procedures. Administered as a continuous IV infusion. Close monitoring for signs of bleeding is required throughout treatment.
