Demecarium Bromide
Demecarium Bromide is a cholinesterase inhibitor ophthalmic solution used to reduce intraocular pressure in glaucoma and to treat strabismus (misaligned eyes). It works by blocking the enzyme acetylcholinesterase, allowing acetylcholine to accumulate and cause pupil constriction, which improves fluid drainage from the eye.
Anticholinesterase eye drops for glaucoma and eye muscle disorders that work by constricting the pupil to reduce eye pressure.

Demecarium Bromide is primarily used for chronic open-angle glaucoma, secondary glaucoma, and as an alternative to surgery for convergent strabismus (crossed eyes) in adults. It may also be used for accommodative esotropia in children.
As a reversible cholinesterase inhibitor, Demecarium Bromide blocks acetylcholinesterase enzymes in the eye, preventing the breakdown of acetylcholine. This allows acetylcholine to accumulate and stimulate muscarinic receptors, causing pupillary constriction (miosis) and ciliary muscle contraction, which opens the trabecular meshwork and improves aqueous humor drainage, thereby lowering intraocular pressure.
Demecarium Bromide is typically reserved for cases unresponsive to other glaucoma medications due to its significant side effect profile, including brow ache, blurred vision, myopia, and potential retinal detachment risk. It should be used with caution in patients with asthma, heart conditions, or seizures. Concurrent use with other cholinergic or anticholinergic agents may alter effectiveness. Pregnant women and nursing mothers should avoid this medication.
