Treatment of lymphatic filariasis caused by Wuchereria bancrofti, Brugia malayi, and Brugia timori; loiasis (Loa loa infection); and tropical pulmonary eosinophilia syndrome. Often used alongside albendazole or ivermectin in public health control programs.

The exact mechanism is not fully elucidated, but DEC is believed to inhibit prostaglandin synthesis and alter microfilarial membranes, making parasites more susceptible to host immune clearance. It immobilizes microfilariae and may have effects against adult worms.

Death of microfilariae can trigger inflammatory reactions including fever, headache, and malaise. Contraindicated in onchocerciasis due to risk of worsening eye disease. In loiasis patients, rapid killing of Loa loa microfilariae can cause severe encephalopathy. Should be used with caution and under medical supervision in endemic areas.
