Donidalorsen

Published: July 26, 2026Updated: July 26, 2026

Donidalorsen is an antisense oligonucleotide (ASO)-GalNAc conjugate designed to reduce plasma kallikrein activity by degrading prekallikrein (PKK) mRNA, thereby decreasing bradykinin production. It is used for the treatment of hereditary angioedema (HAE), a rare genetic disorder characterized by recurrent episodes of severe swelling. By targeting the kallikrein-bradykinin pathway, donidalorsen helps prevent or reduce the frequency and severity of HAE attacks.

Quick summary

Donidalorsen is an ASO therapy that treats hereditary angioedema by reducing plasma kallikrein activity through targeted mRNA degradation, decreasing bradykinin-mediated swelling attacks.

Donidalorsen is indicated for the routine prevention of hereditary angioedema (HAE) attacks in adults and adolescents. HAE is caused by C1 inhibitor deficiency or dysfunction, leading to uncontrolled plasma kallikrein activity and excessive bradykinin release, which causes painful and potentially life-threatening swelling episodes affecting the airways, extremities, face, and abdomen.

Donidalorsen is an ASO conjugated to GalNAc (N-acetylgalactosamine), which facilitates targeted delivery to hepatocytes. Once inside cells, it binds to prekallikrein (PKK) messenger RNA (mRNA) and recruits RNase H1 to cleave and degrade the mRNA, preventing translation into PKK protein. Reduced PKK levels decrease plasma kallikrein activity, which in turn reduces bradykinin generation and prevents angioedema attacks.

Donidalorsen is administered via subcutaneous injection. Common adverse reactions may include injection site reactions. As with other ASO therapies, liver function monitoring may be required. Safety and efficacy in pediatric patients under 12 years have not been established. Patients should have a documented diagnosis of HAE before initiating therapy. Emergency treatment for acute HAE attacks should still be available.
