Severe sepsis (sepsis associated with acute organ dysfunction) in adults at high risk of death. It was specifically indicated for patients with APACHE II scores ≥25 or multiple organ failure.

As a recombinant form of activated protein C, it exerts anticoagulant effects by inhibiting factors Va and VIIIa. It also has anti-inflammatory properties, inhibits leukocyte activation, and promotes fibrinolysis, helping to reduce the dysregulated coagulation cascade seen in severe sepsis.

Withdrawn from market globally in 2011 (PROWESS-SHOCK trial showed no significant mortality benefit). Contraindicated in patients with active bleeding, recent stroke, or severe head trauma. Increased risk of bleeding complications during treatment. Required careful monitoring for bleeding events.
