Epkinly is approved for the treatment of relapsed or refractory diffuse large B-cell lymphoma (DLBCL), high-grade B-cell lymphoma, primary mediastinal large B-cell lymphoma, and follicular lymphoma (Grade 1-3a) in adults who have received at least two prior systemic therapies.

Epkinly is a bispecific CD20-directed CD3 T-cell engager. It simultaneously binds to CD20 proteins on the surface of B-cells (including cancer cells) and CD3 proteins on T-cells, bringing them into close proximity so that the T-cells can recognize and destroy the malignant B-cells.

Epkinly is administered as a subcutaneous injection, typically in increasing doses to help minimize side effects. Serious risks include cytokine release syndrome (CRS), immune effector cell-associated neurotoxicity syndrome (ICANS), serious infections, and neutropenia. Due to the risk of CRS, it is often initiated with a step-up dosing schedule and patients should be monitored closely during and after administration. A REMS (Risk Evaluation and Mitigation Strategy) program is in place for this medication.
