Eplerenone

Published: July 26, 2026Updated: July 26, 2026

Eplerenone is a mineralocorticoid receptor antagonist used primarily to treat heart failure after heart attack and high blood pressure. By blocking aldosterone, it helps reduce fluid retention and lowers blood pressure while preserving potassium levels.

Quick summary

A potassium-sparing diuretic that treats heart failure and hypertension by blocking aldosterone's effects on the kidneys.

Eplerenone is used to treat heart failure (particularly after myocardial infarction) and essential hypertension. It is often prescribed when other blood pressure medications are insufficient or in combination with other heart failure therapies.

Eplerenone selectively binds to mineralocorticoid receptors in the kidneys, blocking aldosterone from binding. This prevents aldosterone-mediated sodium and water reabsorption, reducing blood volume and blood pressure while retaining potassium.

Regular monitoring of potassium levels and kidney function is essential, as hyperkalemia is a risk. Eplerenone is contraindicated in patients with severe kidney impairment, type 2 diabetes with proteinuria, or those taking potassium-sparing diuretics or ACE inhibitors. Side effects may include dizziness, fatigue, and elevated potassium.
