Fludrocortisone

Published: July 26, 2026Updated: August 25, 2026

Fludrocortisone is a synthetic mineralocorticoid used primarily as replacement therapy in conditions where the adrenal glands do not produce adequate natural corticosteroids, most notably Addison's disease. It works by promoting sodium retention and potassium excretion in the kidneys, helping to maintain proper electrolyte and fluid balance in the body. This medication is almost always used in combination with a glucocorticoid to provide complete adrenal hormone replacement.

Quick summary

Fludrocortisone is a mineralocorticoid used to replace missing adrenal hormones in Addison's disease and related conditions by helping the body retain sodium and excrete potassium.

Primary uses include adrenal insufficiency (Addison's disease), salt-losing congenital adrenal hyperplasia, and sometimes orthostatic hypotension. It provides essential mineralocorticoid replacement when the adrenal glands cannot produce adequate hormones on their own.

Fludrocortisone acts on the kidneys to increase sodium reabsorption in the distal tubules and collecting ducts while simultaneously increasing potassium excretion. This helps maintain normal blood pressure, electrolyte balance, and fluid volume. It has minimal glucocorticoid activity compared to its mineralocorticoid effects.

Blood pressure and electrolyte levels (especially sodium and potassium) must be monitored regularly. Dosage must be carefully individualized. Common side effects include fluid retention, hypertension, and low potassium. Should not be stopped abruptly as this can precipitate adrenal crisis. Often combined with a glucocorticoid like hydrocortisone for complete adrenal replacement therapy.
