Treatment of CMV retinitis in patients with AIDS and other immunocompromised states. Also indicated for acyclovir-resistant herpes simplex virus infections when other treatments are ineffective or contraindicated.
Foscarnet is a pyrophosphate analogue that inhibits viral DNA polymerase by blocking the pyrophosphate binding site, preventing the incorporation of nucleotides into viral DNA and halting viral replication.
Administered exclusively by intravenous infusion due to poor oral bioavailability. Requires careful monitoring of renal function, hydration status, and electrolyte levels (calcium, magnesium, potassium) due to risk of nephrotoxicity and metabolic disturbances. May cause seizures in some patients. Regular ophthalmologic monitoring is needed for CMV retinitis patients.
