Used to reduce methotrexate toxicity in patients receiving high-dose methotrexate therapy, and in combination with 5-fluorouracil for the palliative treatment of metastatic colorectal cancer.

Provides reduced folate cofactors that bypass the dihydrofolate reductase enzyme blocked by methotrexate, allowing normal cellular metabolism. In 5-FU regimens, it enhances 5-FU's inhibition of thymidylate synthase.

Must be administered only after methotrexate exposure, not before or concurrently. Requires careful monitoring of renal function and blood counts. Should not be used for pernicious anemia or other megaloblastic anemias caused by folate deficiency. Contraindicated with intrathecal methotrexate.
