Carmustine Implant

Published: July 26, 2026Updated: July 26, 2026

Gliadel is a biodegradable wafer implant containing the chemotherapy agent carmustine (BCNU) that is surgically placed into the brain during tumor removal surgery. It delivers localized chemotherapy directly to the tumor site, allowing high drug concentrations at the target while minimizing systemic exposure. This medication is used as an adjunct treatment for certain malignant brain tumors in patients undergoing surgery.

Gliadel is indicated for the treatment of newly diagnosed high-grade malignant glioma (including glioblastoma multiforme) and recurrent glioblastoma multiforme. It is used as an adjunct to surgery in patients with these aggressive brain tumors.

Gliadel wafers contain carmustine, an alkylating chemotherapy agent that interferes with DNA and RNA synthesis in rapidly dividing cancer cells. When implanted directly into brain tissue at the tumor site, the wafers gradually release the drug over several weeks, providing sustained local chemotherapy that bypasses the blood-brain barrier.

Gliadel wafers are for surgical implantation only and cannot be substituted for systemic chemotherapy. Risks include increased intracranial pressure, cerebral edema, seizures, and wound healing complications. Patients should be monitored closely for neurological changes post-implantation. The wafers are not removable once placed, and healthcare providers should avoid direct skin contact with the wafers during handling to prevent skin irritation.
