Emicizumab

Published: July 26, 2026Updated: July 26, 2026

Hemlibra (emicizumab-kxwh) is a prescription medication used for routine prophylaxis to prevent or reduce the frequency of bleeding episodes in adults and children with hemophilia A (congenital factor VIII deficiency), both with and without factor VIII inhibitors. It is a bispecific monoclonal antibody that mimics the function of factor VIII by simultaneously binding to factor IXa and factor X, thereby restoring the impaired clotting cascade in people with hemophilia A. This medication is administered as a subcutaneous (under the skin) injection, typically given once weekly, every two weeks, or every four weeks based on the prescribed dosing schedule.

Quick summary

Hemlibra is a prophylactic treatment for hemophilia A that works as a bispecific antibody to help restore normal blood clotting, given as a subcutaneous injection.

Hemlibra is indicated for routine prophylaxis to prevent or reduce the frequency of bleeding episodes in patients with hemophilia A (congenital factor VIII deficiency), including those with acquired factor VIII inhibitors (neutralizing antibodies). It may be used in all age groups.

Hemlibra is a bispecific monoclonal antibody that simultaneously binds to factor IXa and factor X, mimicking the cofactor function of endogenous factor VIII. This bridges the activation of factor X by factor IXa, thereby restoring the functionality of the impaired intrinsic tenase complex and facilitating normal blood clotting.

Hemlibra is administered via subcutaneous injection only—it is not given intravenously. Patients or caregivers can be trained to administer it at home. Important safety considerations include the risk of thrombotic microangiopathy and thrombotic events when used with activated prothrombin complex concentrate (aPCC), and it does not correct underlying factor VIII deficiency, so it should not be used to treat acute bleeding episodes. Regular monitoring by a healthcare provider is required.
