Isoptocarpine is primarily used to treat open-angle glaucoma and ocular hypertension. It may also be used to produce miosis (pupil constriction) and to counteract the effects of mydriatic agents.

Isoptocarpine contains pilocarpine, a direct-acting muscarinic agonist. It stimulates muscarinic receptors in the smooth muscle of the iris sphincter and ciliary body, causing pupil constriction and increased outflow of aqueous humor through the trabecular meshwork, thereby reducing intraocular pressure.

Common side effects may include blurred vision, brow ache, and myopia (difficulty seeing at distance). Patients with asthma, bradycardia, or certain eye conditions may need special caution. Avoid driving or operating machinery until vision clears. As a brand-name medication, availability and cost may vary compared to generic alternatives.
