Istradefylline
Istradefylline is an adenosine A2A receptor antagonist used as an adjunct treatment for Parkinson's disease. It is prescribed alongside levodopa/carbidopa therapy for patients experiencing motor fluctuations or 'off' episodes when standard treatment provides inadequate symptom control. By blocking adenosine A2A receptors in the brain, it helps reduce 'off' time and improve motor function.
Istradefylline is an adjunct Parkinson's disease medication that blocks adenosine A2A receptors to reduce motor fluctuations when used with levodopa.

Adjunctive treatment for Parkinson's disease to reduce 'off' episodes in patients on levodopa/carbidopa therapy who experience motor fluctuations or inadequate symptom control with levodopa alone.
Istradefylline is a selective adenosine A2A receptor antagonist. While its precise therapeutic mechanism in Parkinson's disease is unknown, blocking A2A receptors in the striatum is believed to modulate dopaminergic signaling and reduce inhibition of motor control, thereby complementing the effects of levodopa.
Should be taken once daily, either with or without food. Common side effects include dyskinesia, dizziness, nausea, and insomnia. Patients should be monitored for worsening of dyskinesia when initiating therapy. Not recommended for use in pregnant or breastfeeding individuals unless clearly needed.
