Isturisa is indicated for the treatment of endogenous Cushing's syndrome (specifically Cushing's disease) in adult patients who are not candidates for pituitary surgery or have not responded adequately to prior pituitary surgery. It is specifically approved for use in the United States and Europe for this indication.
Isturisa is a cortisol synthesis inhibitor that works by blocking the enzyme 11β-hydroxylase, which plays a critical role in the production of cortisol in the adrenal glands. By inhibiting this enzyme, the medication reduces the amount of cortisol produced, helping to normalize elevated cortisol levels characteristic of Cushing's disease.
Regular monitoring of cortisol levels, liver function tests, and electrolytes (particularly potassium) is essential during treatment. The medication may cause adrenal insufficiency, hypokalemia, and QT prolongation. Common side effects include fatigue, nausea, headache, and edema. Dose adjustments are based on individual cortisol levels and tolerability. Female patients of childbearing potential should use effective contraception during treatment. Never discontinue abruptly, as this requires gradual dose tapering under medical supervision.
