Janumet is indicated for improving blood sugar control in adults with type 2 diabetes mellitus, particularly when glycemic goals are not achieved with diet and exercise alone, or when already taking the individual components.

Janumet combines two mechanisms: sitagliptin (a DPP-4 inhibitor) works by increasing incretin hormones to stimulate insulin release and reduce glucagon in a glucose-dependent manner, while metformin (a biguanide) decreases liver glucose production, reduces intestinal glucose absorption, and improves insulin sensitivity.

Janumet carries a boxed warning for lactic acidosis, a rare but serious metabolic complication associated with the metformin component. It should not be used in patients with severe kidney impairment, metabolic acidosis, or diabetic ketoacidosis. Common side effects include gastrointestinal symptoms such as nausea, diarrhea, and abdominal discomfort. The medication should be taken with meals to reduce GI side effects.
