Lonsurf is indicated for the treatment of adults with metastatic colorectal cancer who have previously been treated with fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, and if applicable, anti-VEGF therapy and anti-EGFR therapy. It is also used for metastatic gastric or gastroesophageal junction adenocarcinoma in patients who have received prior treatment with at least two chemotherapy regimens.

Lonsurf contains two active ingredients: trifluridine and tipiracil. Trifluridine is incorporated into DNA during cell replication, interfering with DNA synthesis and inhibiting cell proliferation. Tipiracil is a thymidine phosphorylase inhibitor that prevents the breakdown of trifluridine, thereby maintaining effective drug levels in the body.

Lonsurf may cause serious side effects including bone marrow suppression (anemia, neutropenia, thrombocytopenia), gastrointestinal symptoms (nausea, vomiting, diarrhea), and fatigue. Regular blood monitoring is required during treatment. The medication should be taken with food and dosing is based on body surface area. Patients should avoid pregnancy during treatment as it may cause harm to a developing fetus.
