Treatment of CMV infection in adult patients who have received solid organ or hematopoietic stem cell transplants and whose infection is refractory or resistant to conventional antiviral therapies such as ganciclovir, valganciclovir, cidofovir, or foscarnet.
Maribavir inhibits the CMV UL97 protein kinase, an enzyme essential for viral DNA replication, virion assembly, and egress of viral particles from infected cells, thereby preventing viral replication.
Common adverse reactions include taste disturbances (dysgeusia), nausea, diarrhea, vomiting, and fatigue. Drug interactions may occur with immunosuppressants and other medications metabolized through CYP3A pathways. Regular monitoring of CMV viral load and renal/hepatic function is recommended during treatment. Dosage is typically 400 mg (2 tablets) taken twice daily.
