Treatment of symptomatic obstructive hypertrophic cardiomyopathy (HCM) in adults who have NYHA Class II-III symptoms, to improve exercise capacity and symptoms.

Mavacamten is an allosteric and reversible inhibitor selective for cardiac myosin. It modulates the number of myosin heads that can enter 'on actin' (power-generating) states, reducing the probability of force-producing (systolic) and residual (diastolic) cross-bridge formation, thereby decreasing excessive cardiac contractility.

Requires echocardiographic monitoring for left ventricular ejection fraction (LVEF). Use is contraindicated in patients with LVEF <55%. May cause heart failure exacerbation in those with markedly elevated LVEF gradients at rest. Pregnancy should be avoided during treatment due to potential fetal harm. Drug interactions with CYP2C19 and CYP3A4 inhibitors require dose adjustment.
