Treatment of schizophrenia and other psychotic disorders, management of severe pain (as an adjunct analgesic, particularly in palliative care), relief of nausea and vomiting, and control of agitation and anxiety in end-of-life care.
Methotrimeprazine is a phenothiazine antipsychotic that works primarily by blocking dopamine D2 receptors in the brain. It also antagonizes histamine H1 receptors, alpha-1 adrenergic receptors, and muscarinic acetylcholine receptors, contributing to its sedative, antiemetic, and anxiolytic effects.
Common side effects include significant sedation, orthostatic hypotension (low blood pressure upon standing), dry mouth, and extrapyramidal symptoms. It should be used with caution in elderly patients due to increased risk of adverse effects and mortality. Alcohol and other CNS depressants may enhance sedative effects. May cause photosensitivity; avoid prolonged sun exposure.
