Mechanism of action
Molecular mechanism of drug action
Treats inflammatory and autoimmune conditions including rheumatoid arthritis, lupus, severe asthma exacerbations, allergic reactions, multiple sclerosis relapses, certain skin conditions (such as severe psoriasis and dermatitis), and adrenal insufficiency. Also used as adjunctive therapy in some cancer treatments.
As a glucocorticoid, methylprednisolone binds to intracellular corticosteroid receptors and inhibits phospholipase A2, which reduces the production of inflammatory mediators including prostaglandins and leukotrienes. It also suppresses immune cell activity and reduces capillary permeability.
Long-term use requires gradual dose tapering to avoid adrenal insufficiency. Common side effects include increased appetite, mood changes, elevated blood sugar, weight gain, and increased infection risk. Live vaccines should be avoided during treatment. May interact with drugs metabolized by CYP450 enzymes. Not for use in patients with systemic fungal infections.
