Metolazone is primarily used to treat fluid retention (edema) caused by conditions such as congestive heart failure, kidney dysfunction, and liver cirrhosis. It is also prescribed for managing essential hypertension (high blood pressure) either alone or in combination with other antihypertensive medications.
Metolazone works by blocking sodium reabsorption in the distal convoluted tubules of the kidneys. This prevents the kidneys from retaining excessive sodium and water, leading to increased urine output (diuresis). The loss of sodium and water helps reduce fluid buildup in the body and decreases blood pressure.
Metolazone may cause electrolyte imbalances, particularly low potassium (hypokalemia), low sodium (hyponatremia), and low magnesium. Patients should have their electrolyte levels and kidney function monitored regularly. It may increase blood sugar, uric acid levels, and cause photosensitivity. Those with sulfa drug allergies should use caution. Alcohol and other diuretics may enhance blood pressure lowering effects and increase the risk of dehydration and dizziness upon standing.
