Published: July 26, 2026Updated: July 26, 2026

Metopirone (metyrapone) is a diagnostic medication primarily used to test hypothalamic-pituitary-adrenal (HPA) axis function. It works by temporarily blocking cortisol production, allowing doctors to determine if adrenal insufficiency is caused by primary adrenal failure or secondary to pituitary/hypothalamic dysfunction. It is typically used in hospital or clinical settings under medical supervision.

Used as a diagnostic test for adrenal insufficiency to differentiate between primary adrenal failure (Addison's disease) and secondary insufficiency due to pituitary or hypothalamic dysfunction. May also be used in managing Cushing's syndrome in certain cases.

Inhibits 11β-hydroxylase, the enzyme responsible for converting 11-deoxycortisol to cortisol in the adrenal gland. By blocking this step, it reduces cortisol production and allows assessment of the pituitary's ability to respond by increasing ACTH.

Should only be administered in settings where cortisol levels can be measured. Contraindicated in patients with primary adrenal insufficiency as it may cause acute adrenal crisis. Common side effects include nausea, dizziness, and headache. Must be discontinued before and during surgery or acute stress.
