Monomethyl fumarate is an immunomodulating medication used to treat relapsing forms of multiple sclerosis (MS), including relapsing-remitting MS and active secondary progressive MS. It is taken orally and works by activating the Nrf2 pathway, which is involved in cellular antioxidant and anti-inflammatory responses, though its exact mechanism in MS is not fully understood.

Treatment of relapsing forms of multiple sclerosis, including relapsing-remitting MS (RRMS) and active secondary progressive MS (SPMS) in adults.
Activates the Nrf2 (Nuclear factor erythroid-derived 2-like 2) pathway, which triggers antioxidant and anti-inflammatory gene expression. The complete therapeutic mechanism in MS is not fully characterized.
Common side effects include flushing, gastrointestinal symptoms (diarrhea, nausea, abdominal pain), and lymphopenia. Liver function monitoring may be required. May increase risk of infections. Contraindicated in patients with known hypersensitivity. Women of childbearing potential should use effective contraception during treatment.
