Motixafortide

Published: July 26, 2026Updated: July 26, 2026

Motixafortide is a CXCR4 receptor antagonist used to mobilize hematopoietic stem cells from the bone marrow into the bloodstream for collection and subsequent autologous transplantation in patients with multiple myeloma or lymphoma. By blocking the CXCR4-SDF-1α/CXCL12 axis, it disrupts stem cell retention in the bone marrow niche, allowing these cells to circulate for harvesting. This medication is administered as a subcutaneous injection typically given prior to stem cell collection procedures.

Quick summary

Motixafortide is a CXCR4 inhibitor used to mobilize stem cells for collection before autologous transplantation in blood cancer patients.

Motixafortide is indicated for mobilizing hematopoietic stem cells from bone marrow to peripheral blood for collection and subsequent autologous transplantation in patients with multiple myeloma and lymphoma.

Motixafortide is a selective CXCR4 receptor antagonist that blocks the binding of stromal-derived factor-1α (SDF-1α/CXCL12), the cognate ligand for CXCR4. This disruption of the CXCR4-SDF-1α interaction releases hematopoietic stem and progenitor cells from their bone marrow niche into the peripheral circulation.

Motixafortide is administered as a subcutaneous injection, typically 0.24 mg/kg once daily. Common adverse effects include injection site reactions, transient leukocytosis, and musculoskeletal pain. It should be used with granulocyte colony-stimulating factor (G-CSF) for optimal stem cell mobilization. Patients should be monitored for potential effects on cardiac conduction and hypersensitivity reactions.
